Student Medical Procedures

A Medical Procedure Request/Waiver Form MUST be filled out for students requiring short-term and long-term medication (including inhalers in backpacks). Please return the completed form to the office and they will determine where the medication should be kept and who should administer it.

Medication can not be transported to and from school on the bus. Please make arrangements to have the Medical Procedure form and medication to be brought in to the front office by a Parent or Guardian.

Any medication sent to the school without a completed form WILL NOT be administered to the child.
